Maybe the island somehow slows down the aging process. During season 4 it is explained that time is different on the island, when the doctor from the freighter washes up ashore on the island before he is murdered on the boat. I think the problem has to lie in the time differential on the island. If you think about it, that island could exist in any time throughout the history of the island...at any time. We don't know what that does to your physical "INSIDES" we do know, John can walk, Rose's cancer can be cured, and pregnant women's inside look like 70 year olds.
