When you die your name gets crossed off, when all the candidates are dead, Jacob begins working on bringing another group to the island. How or why the numbers are what they are I haven't got a clue. On another post I had the thought that Kate isn't on the list (Mikhail also said that she wasn't) because she didn't follow Jacob's path and stop stealing - she was a very naughty girl ;) so maybe she 'excused' herself from becoming a candidate by not being the person Jacob wanted.
